These lumps, known as tonsilloliths, or calculus of the tonsil, or tonsil stones, can range in size from tiny little bits to pea sized ones and they have a tendency of staying lodged in the area of the tonsils for some time, until they are dislodged finally. One method is to use a stainless long handled spoon 7.5 inches. You would have to push the stone out from beneath. Using this method works easier if the stone is older and harder. It's also possible to use tweezers to pluck the stone out. The use of pulsating irrigation to clear out the crypts of built up debris may also help (using an adjustable unit over a low pressure setting or a needleless Monoject syringe in order to avoid damaging tissue.) Alternately medicine droppers (especially one with a curved tip) are utilized to suck out the more compact stones. Using such a medicine dropper to first irrigate the area with saline which may help to bring the tonsil stone to the surface from which it can then be more easily sucked out with the dropper. To get relief from gagging people often consider using everyday things, like toothpicks, as small tools instead of using a much larger finger. This is simply not advised because of the danger of the short light-weight toothpick/object being released and breathed into the throat. Use extreme caution when attempting any of the methods in the list above. The tonsils and throat are incredibly delicate, and can be damaged or inflamed quite easily.
